Statements of Economic Interests

Q: Who is required to file a Statement of Economic Interest Form?
A: Elected officials, appointed officials and employees of Local Units of Government. Employee names are provided to the Clerk’s office by the Unit of Government.
 
Q: When is the Statement of Economic Interest due?
A: May 1 each year. A $15 late filing fee is assessed after May 1. 
 
Q: Where can I file a Statement of Economic Interests Form in person?
A: If you are required to file because your name was submitted by a unit of government, a Statement of Economic Interests should be filed online with the information provided in the letter we sent. Statements may also be filed in person at 69 W. Washington, Fifth Floor, Chicago. Filings are currently not accepted the five suburban mini-centers. NOTE: If you file in person, you will be asked to complete the form on a computer terminal made available to you.
 
Q: Can the Statement of Economic Interest form be mailed or faxed?
A: The form should be completed online with the information you received in our letter. If you are a candidate, you may mail your form to us.
 
Q: I’m no longer with my previous employer. How do I remove my name from your list?
A: Contact the Unit of Government you previously worked for and ask them to take you off the list.

Lobbyist Filings

Q: How do I become a Lobbyist in Cook County?
A: The registration forms must be completed and submitted to the Clerk’s office. The required registration fee is $350. Note: Facsimiles are not acceptable.
 
Q: How often am I required to file?
A: You must register by January 20 every year. The registration fee of $350 is required at that time. The expenditure report is required twice a year by January 20 and July 20.
 
Q: Can I amend my forms?
A: Yes, registration and expenditure forms can be amended.

Campaign Disclosures

Q: When is the Statement of Organization (D-1) required?
A: The D-1 must be filed within 10 business days of reaching $3,000 in contributions or expenditures.
 
Q: How often is the D-1 required?
A: A new D-1 is required when changes are made to your organization (officers, changes of address, banks etc).
 
Q: Where do I submit the D-1?
A: The D-1 can be faxed to (312) 603-9787, submitted in person or mailed to our office at 69 W. Washington, Fifth Floor, Chicago, IL 60602.
 
Q: When am I required to file the semi-annual report?

Q: How can I get the Campaign Disclosure calendar?
A: The Campaign Disclosure calendar is issued by the State Board of Elections. Note: The State Board of Election holds classes for electronic filing of Campaign Disclosure reports at:
